Please fill out the form below to select an appointment time.  Appointments are currently being offered only via telehealth visits, we will call you at your scheduled appointment time.  If you are a new patient to the PA medical marijuana program please be sure to register with the state at http://www.medicalmarijuana.pa.gov to obtain a state ID number.  After you obtain a physician certification you will be required to pay $50 to the state to receive your card.  More information about the program can be found here.
